How To: Insurance Reimbursement

After receiving your insurance claim forms from The Center:

1. Fill out the top portion with your personal information. IMPORTANT: Do not sign in the following box:

2. Mail your insurance form, letter of medical necessity and any additional paperwork to your insurance company.

3. If you do not hear back within 3 weeks, call to check in.

If you need more specific assistance though, please email insurance@breastfeedingcenter.org and we will help troubleshoot.

*If you have had a consultation, but haven’t received an insurance form, please email us at insurance@breastfeedingcenter.org (keep in mind that insurance forms for home visits are sent within 30 days of your visit).

We work with the following insurance companies*:

Aetna: We are in network with Aetna. This means that you can be seen by a lactation consultant at the Center at no cost to you (barring grandfathered plans). Just make sure you bring your insurance card with you at the time of your visit. In the unlikely event the claim is denied (for example, you have a grandfathered plan or your employer opted out of the ACA), you will be billed for the consultation.

United: We are in network with United. This means that you can be seen by a lactation consultant at the Center at no cost to you (barring grandfathered plans). Just make sure you bring your insurance card with you at the time of your visit. In the unlikely event the claim is denied (for example, you have a grandfathered plan or your employer opted out of the ACA), you will be billed for the consultation.

BCBS (all non-federal plans): We give you the coded forms to submit to BCBS for reimbursement. Some plans under BCBS also require a pre-authorization, so please call BCBS before your appointment to get the necessary paperwork in place. They may ask you for some information about our Center, most of which can be found in our Insurance FAQ.

BCBS (Federal): We give you the coded forms to submit to BCBS for reimbursement. Some plans under Federal BCBS also requires a pre-authorization, so please call BCBS before your appointment to get the necessary paperwork in place. They may ask you for some information about our Center, most of which can be found in our Insurance FAQ.

Cigna: We give you the coded forms to submit to Cigna for reimbursement. Many plans within Cigna require a pre-authorization, so please call Cigna before your appointment to get the necessary paperwork in place. They may ask you for some information about our Center, most of which can be found in our Insurance FAQ

Kaiser: Since Kaiser does have a lactation consultant on staff, it is unlikely that they will reimburse for a consultation with the Center. However, we will provide you with the coded forms to submit to them for reimbursement just in case. If you have been reimbursed by Kaiser, please let us know.
