HIPAA Notice of Privacy Practice
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
The Breastfeeding Center of Greater Washington (BCGW) is required by law to abide by the Health Insurance Portability and Accountability Act of 1996 (HIPAA), which is designed to protect the privacy of information related to your healthcare, and how it is used and shared.
BCGW may use the protected health information (PHI) you share with us to aid in your healthcare management, bill you and/or your insurance provider for services, conduct research (in which only de-identified information will be used and approval will first be obtained from an Institutional Review Board), and comply with both State and Federal law (e.g. in response to a subpoena).
You have the right to:
Request a copy of your chart
Ask us to correct any errors made on your chart
Request that we communicate with you in a specific way (e.g. cell phone, home phone, email, etc)
Indicate specific individuals with whom you would like us to share your information
Ask us to limit the PHI we use or share
We have the right to deny this request if we feel it would affect your care or if doing so would violate the Code of Professional Conduct for IBCLCs.
Receive a list of those with whom we have shared your PHI.

If you feel the matter has not been adequately addressed you may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.
